What is a Children's Health Home?

A Health Home is a community-based care management service model whereby all of an individual's caregivers communicate with one another so that all social, physical, and behavioral needs are addressed in a comprehensive manner. This is done through a dedicated Care Manager who oversees and provides access to all of the services an individual needs to ensure they receive everything necessary to stay healthy, out of the emergency room and out of the hospital.
ELIGIBILITY
All children/youth must meet the following eligibility requirements to be considered for enrolloment: 
1.  The child/youth is under the age of 21, enrolled in Medicaid and resides in a CHHUNY county.
2.   The child/youth must meet the NYS DOH eligibility. 
3.   Child/youth has significant behavioral, medical or social risk factors which can be addressed through care management.

Who is CHHUNY?

The Children’s Health Homes of Upstate New York, LLC (CHHUNY) was established to plan for and operate as a Health Home for Children. CHHUNY is using a model of care management based upon the belief that every family has strengths and that families are the natural and most effective drivers of their care. This model also ensures that families' voice and choice is centered throughout every stage of the care management process.
